Family Advocacy and  Consultation Services

DSPD Support Coordination
Family Advocacy is a social services agency located in northern Utah and provides services to individuals with cognitive, developmental, physical, and mental health disabilities, to brain injury survivors, and those who support them.
Family Advocacy provides state certified support coordinator services to individuals receiving services from the Division of Services of People with Disabilities (DSPD) and those who qualify for and receive medicaid waiver services (Community Supports Waiver).

We will assist you in developing services, finding qualified providers, and developing and monitoring support plans, based upon your needs and wishes, and will coordinate and monitor the services and supports provided.

We offer services throughout Utah and have experience working with a variety of DSPD contracted providers. Our support coordinators can also assist you with self-administered services and equipment purchases.
